2009 - 2010 Sports Information
 
 
In order to compete in an APMS Sport for the 2009-10 school year, you will need to submit the following forms to the Athletic Office:
  • Current Physical (MHSAA form must be completed after May 15th, 2009)
  • Signed Athletic Code of Conduct
  • Emergency Contact Form
  • Signed Insurance Waiver Form
  • Participation Fee submitted once on team roster
